Work Environment

This position is based onsite at our home care facility, where you will collaborate with a multidisciplinary team of healthcare professionals, including cardiologists, nurses, dietitians, and physical therapists. Key Responsibilities

• Conduct detailed patient assessments and maintain accurate medical records that adhere to regulatory standards, including electronic health records management.
• Collaborate with multidisciplinary teams to design and implement personalized care plans that align with best practices in cardiovascular treatment.
• Engage in routine patient follow-up visits for monitoring treatment progress, ensuring continuity of care and adjustments to management plans as necessary.
• Prescribe appropriate medications and therapies based on individual patient needs, complying with established clinical guidelines and protocols.
• Manage chronic cardiovascular conditions such as hypertension, heart failure, and arrhythmias, employing evidence-based practices to improve patient health and prevent complications.
• Educate patients and their families about heart health, risk factors, and self-management strategies to empower them in their health decisions.

Patient Population / Client Focus

You will have the opportunity to work with a diverse patient population, including individuals with varying degrees of cardiovascular health challenges. Our patients range from those experiencing acute cardiac events to those managing chronic conditions. Qualifications

• Master's degree in Nursing from an accredited institution.
• Current Nurse Practitioner license in the state of New York, with certifications in relevant areas of cardiology preferred.
• Minimum of two years of clinical experience in cardiology or a related field, showcasing a strong understanding of cardiovascular health.
• Demonstrated ability to work independently and collaboratively within a multidisciplinary team setting.

Preferred Experience

• Previous experience in a home care or outpatient cardiology setting is a plus.
• Familiarity with electronic medical record (EMR) systems and telehealth practices.
• Additional certifications in advanced cardiovascular life support (ACLS) or similar disciplines.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from $150,000 to $160,000, complemented by a comprehensive benefits package that includes health, dental, and vision insurance, paid time off, and retirement savings options.
