Key Responsibilities

• Conduct thorough assessments of patient health, which includes interviewing residents, performing physical examinations, and reviewing comprehensive medical histories.
• Develop, implement, and oversee patient management plans to ensure continuity and quality of care.
• Collaborate closely with physicians to manage both short-term ailments and long-term health conditions in internal medicine.
• Educate patients and their families about health conditions, treatment plans, and preventive measures.
• Document all patient interactions and care plans accurately in our electronic health record (EHR) system to maintain clear communication across care teams.

Patient Population / Client Focus

Our internal medicine team serves a population of elderly patients who may have complex health needs, including chronic illnesses and multiple co-morbidities. You will be responsible for providing proactive, patient-centered care that emphasizes preventive health measures and the individual needs of each resident. Your expertise and compassionate approach will play a vital role in enhancing the quality of life for our residents.

Qualifications

• A Bachelor’s degree from an accredited Nurse Practitioner or Physician Assistant program.
• A current and active NP/PA license in the state of New York.
• Completion of the Physician Assistant National Certifying Examination (PANCE) for PAs or relevant certification for NPs.
• A minimum of two years of experience in an ambulatory care setting is preferred.

Preferred Experience

While two years of prior experience in an ambulatory care setting is preferred, we welcome candidates with a strong willingness to learn and adapt to our unique patient environment. Experience working with the elderly and familiarity with geriatric medicine will be highly advantageous.

Compensation and Benefits
