Work Environment

Key Responsibilities

• Conduct comprehensive physical examinations, assess health status, and diagnose acute, episodic, or chronic illnesses.
• Prescribe medications and develop personalized treatment plans aligned with patient needs and health goals.
• Communicate effectively with collaborating physicians and specialists to coordinate and enhance patient care.
• Develop and implement management policies and procedures to improve overall patient health outcomes.
• Facilitate referrals to other healthcare professionals and medical facilities as necessary to ensure continuity of care.
• Build rapport and trust with patients and their families to foster adherence to treatment protocols and maintenance of health.

Patient Population / Client Focus

Our patient population primarily consists of seniors and individuals from low-income backgrounds who may face barriers to accessing traditional healthcare facilities. By providing concierge medicine and house call services, you will be instrumental in delivering comprehensive care that is adaptable to the specific challenges faced by these communities. Your role is crucial in enhancing health literacy and facilitating health-related conversations within households that may otherwise feel disconnected from traditional healthcare resources.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification from AANP or ANCC, demonstrating your commitment to professional standards in nursing.
• Active, unrestricted state licensure in New York with eligibility to practice without conditions.
• Active DEA license essential for prescribing medications and treatments.
• Authorization to work in the United States.
• Willingness to work in a flexible setting where adaptability is key.

Preferred Experience

• Experience in geriatric care or working with underserved communities preferred.

Compensation and Benefits
