Heart Care Center PC is seeking dedicated and compassionate Full-Time Nurse Practitioners (NPs) to join our healthcare team in Poughkeepsie, New York. As a key member of our esteemed concierge medicine practice, you will be pivotal in diagnosing and treating various health conditions while promoting proactive health practices to our patients. Work Environment

This role is primarily onsite at our Poughkeepsie facility and operates within a supportive and collaborative environment. You will work a standard schedule of 40 hours per week from Monday to Friday, with evenings up to 5:00 PM. Our practice emphasizes teamwork, communication, and a shared commitment to providing high-touch care, making it an ideal setting for NPs who wish to focus on patient-centered medical service.

Key Responsibilities

As a Nurse Practitioner at Heart Care Center PC, you will:

• Conduct thorough physical examinations and assess patients' medical histories.
• Order and interpret diagnostic tests, such as blood work and imaging studies, to assist in accurate diagnosis.
• Diagnose acute and chronic health conditions, determining appropriate treatment options tailored to individual patient needs.
• Prescribe necessary medications while adhering to state regulations and best practices.
• Maintain comprehensive and accurate medical records, ensuring all patient interactions are documented appropriately.
• Provide health education to patients and their families, focusing on preventative measures and chronic disease management.
• Collaborate effectively with physicians, fellow Nurse Practitioners, and other healthcare staff to ensure optimal patient care.
• Engage in continuous professional development and research to enhance clinical knowledge and practice.

Patient Population / Client Focus

Qualifications

The ideal candidate will possess the following qualifications:

• A valid New York State Nurse Practitioner license, with certification as an Adult Nurse Practitioner (ANP) or Family Nurse Practitioner (FNP).
• A master’s degree in Nursing, with an emphasis on advanced practice.
• A current DEA number enabling you to prescribe medications.
• Expertise in primary, acute, and chronic care, preferably with experience in a clinic or hospital setting.
• Demonstrable abilities in patient assessment, critical thinking, and evidence-based treatment planning.
• Exceptional communication skills, fostering strong patient-provider relationships.
• A commitment to teamwork, ongoing learning, and maintaining high ethical standards.

Preferred Experience

While not mandatory, candidates with bilingual capabilities will find these skills beneficial in our diverse community. Experience in a concierge medicine setting or familiarity with innovative healthcare delivery models is advantageous.
