Work Environment

This position is primarily based in our outpatient office, where you will work alongside a team of medical professionals committed to vibrant community health. With a structured work schedule averaging 40 hours per week, you will typically operate from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m. Your involvement may also extend to weekends for community outreach events or health fairs, emphasizing our commitment to serving diverse populations outside traditional working hours.

Key Responsibilities

• Perform comprehensive health assessments and screenings for patients including children, adults, and families to identify unique health needs.
• Diagnose and manage both acute and chronic medical issues, providing care plans that integrate preventive measures and health promotion strategies.
• Work collaboratively with local community organizations to develop initiatives focused on health education and prevention, such as workshops on chronic disease management.
• Advocate for patient rights, ensuring equitable access to healthcare services, and providing culturally sensitive care tailored to individual backgrounds and needs.
• Efficiently manage referrals and coordinate follow-up care, guaranteeing continuity of care tailored to individual patient journeys.
• Maintain meticulous medical records in adherence to regulatory and organizational standards, leveraging electronic health record (EHR) systems to enhance efficiency.

Qualifications

Compensation and Benefits
