Work Environment

This role will be conducted in an onsite setting within a school, allowing you to engage directly with students and their families. The work schedule will be Monday to Friday from 9 am to 5 pm, allowing for a structured work-life balance. Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families to identify and address health concerns.
• Diagnose and manage acute illnesses and chronic medical conditions, ensuring a thorough understanding of preventive care and health promotion strategies.
• Develop individualized care plans in collaboration with students, families, and educational staff, ensuring alignments with their unique healthcare needs.
• Engage with community organizations to create and implement relevant health education programs, enhancing health literacy among students and families.
• Advocate for patients' rights to access healthcare services while providing culturally sensitive care tailored to a diverse student population.
• Coordinate referrals and follow-up, ensuring continuity of care and effective transitions between healthcare providers.
• Document patient interactions and medical records accurately, adhering to regulatory standards and maintaining confidentiality.
• Participate in community outreach events and health fairs to promote health awareness and deliver essential screenings.
• Stay informed about trends in community health, evidence-based practices, and applicable legislation to enhance service delivery.

Patient Population / Client Focus

You will work primarily with the school-aged population, along with their families, addressing a wide array of health issues from developmental concerns to chronic conditions. Your role is crucial in promoting preventative health measures, guiding families in accessing necessary resources, and fostering a healthy school environment. Given the unique needs of students, you will be instrumental in bridging gaps in care and ensuring there are supportive services available for all patients.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited nursing program.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Unrestricted Nurse Practitioner (APRN) license in New York State along with a valid DEA# and NPI#.
• Minimum of two years of clinical experience, preferably within primary care or family medicine settings.
• Demonstrated clinical assessment and diagnostic skills, with a proactive approach to patient care.
• Strong proficiency in using electronic health record (EHR) systems to maintain accurate patient documentation.
• Exceptional communication and interpersonal skills, essential for working collaboratively in a diverse environment.
• Able to work independently as well as part of a multidisciplinary team.

Preferred Experience

• Experience in community health or public health, particularly within school-based health systems, will be viewed favorably.
• Established track record of engaging with diverse populations and adapting care practices to meet various cultural needs.
