This position is located onsite at a local school in Poughkeepsie, New York, where you will engage with a vibrant community of students and faculty. Key Responsibilities

• Provide comprehensive medical care to adult and geriatric patients, managing both acute and chronic cardiology-related health issues.
• Conduct detailed patient evaluations, including thorough histories and physical assessments to establish accurate diagnoses.
• Develop and execute individualized care plans tailored to the specific clinical needs and preferences of each patient.
• Perform diagnostic and therapeutic procedures such as EKGs, echocardiograms, and stress tests, utilizing advanced medical equipment specific to cardiac care.
• Prescribe and manage medications, ensuring optimal therapeutic outcomes while monitoring for potential side effects.
• Maintain precise and timely documentation of patient encounters in electronic health records, ensuring compliance with billing and medical record standards.

Patient Population / Client Focus

The primary patient population for this role includes students within the school setting, typically ranging from young adults to elderly individuals who may have acute or chronic cardiac conditions. Your work will focus on educating patients and their families about heart health, promoting lifestyle changes, and intervening early in cardiac issues to improve health outcomes.

Qualifications

• A Master's Degree in Nursing, specifically as a Nurse Practitioner.
• An active Nurse Practitioner license in the state of New York.
• At least two years of clinical experience, preferably in a cardiology-focused environment.
• Demonstrated proficiency in patient assessment, management of cardiac diseases, and treatment options.
• Strong interpersonal and communication skills, enabling effective engagement with patients, families, and multidisciplinary healthcare teams.
• Ability to work independently and uphold a high standard of professionalism and confidentiality.

Preferred Experience

• Experience in a school health or community health setting.

Compensation and Benefits
