How You'll Make an Impact in This Role

• Coordinate the interdisciplinary plan of care from admission through discharge, ensuring seamless transitions and consistent, high-quality patient care.
• Monitor patient progress through ongoing assessments and medical record review to ensure care plans remain appropriate and goal-focused.
• Advocate for patients by securing necessary resources and removing barriers that may delay treatment, discharge, or recovery.
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure timely implementation of the health plan and continuity of care.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What Additional Preferences We're Seeking

• Previous charge or supervisory experience preferred.
• Previous experience in a hospital setting preferred.
