MSW Social Worker

Job Summary We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to provide skilled nursing care, case management, and hospice care. The role involves patient assessments, medication administration, treatment planning, and interdisciplinary collaboration, with a focus on discharge planning. The candidate must have at least 2 years of acute hospital case management experience and be able to work weekends and holidays as required. Responsibilities Provide skilled nursing care including patient assessments and medication administration. Develop and implement treatment plans in collaboration with the interdisciplinary team. Manage case load with a ratio of approximately 1:35 patients. Focus on discharge planning to ensure smooth patient transitions. Cover up to 2 units on weekends as scheduled. Participate in holiday coverage as required. Required Skills & Certifications Minimum 2 years of acute hospital case management experience. Active Registered Nurse license in the state of practice. Clinical assessment, medication administration, and treatment planning skills. Interdisciplinary team collaboration skills. Excellent communication and interpersonal skills. Bachelor's degree in Nursing or related field. Driver's license, car insurance, and automobile for field work (if applicable). Preferred Skills & Certifications Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings. Experience with discharge planning and case management in acute care. Special Considerations Weekend coverage required every other weekend. Must be available for holiday coverage. Return to Office (RTO) restrictions limit time in office to no more than 7 days. Work environment is an acute care facility. Scrub or business casual attire required. Scheduling Work schedule includes weekend shifts every other weekend. Shift assignments may include coverage of up to 2 units on weekends. Holiday shifts as assigned. #J-18808-Ljbffr
