In-Home Primary Care NP — Field & Community Health
Molina Healthcare of Illinois
Molina Healthcare seeks a Nurse Practitioner to provide screening, preventive primary care, and medical services in in-home, community, and nursing facility settings. You will coordinate care, perform exams, order labs, and create medical plans with physician oversight. The role involves travel across the assigned region (up to 120 miles daily) and requires licensure, DEA prescriptive authority, BLS, and EMR proficiency.
