• Valid DEA license and state dental license
• Prior experience in general dentistry, especially with pediatric and adult patients

Other Requirements:

• Must be willing to commit for a minimum of 3 months, with the possibility of extension
• Must satisfy all credentialing requirements, including immunizations, TB testing, and maintaining current certifications
• Active DEA license required prior to start; credentialing process takes approximately 30-60 days
