We're looking for an MDS Coordinator to oversee the assessment process and ensure accurate documentation for optimal care planning and reimbursement. Major Responsibilities
- Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with Federal and State regulations.
- Monitor and document the management of Medicare and Managed Care residents, working closely with the interdisciplinary team.
- Ensure accuracy in resident assessments to support reimbursement and high-quality care.
- Drive innovation by implementing new ideas and process improvements.
- Stay up to date with regulations and best practices to maintain compliance.
Minimum Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing.
- 6+ months of experience as an MDS Coordinator required.
- Experience in Skilled Nursing/ Rehabilitation preferred.
- RAC-CT or RNAC certification preferred.
- A detail-oriented, compassionate professional with a commitment to excellence.
