Clinical Coordinator I Job Type Full-Time Pay Rate $68,000 - $75,000/yr. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low‐ income older persons, many of whom live with mental illness and/or have experienced homelessness. Position Description The Clinical Coordinator I will carry a small caseload and works closely with the Building Manager. Please Note: This is a full-time position (5) days a week on-site. Job Duties & Responsibilities Provide clinical and case management supervision to all social work and case management staffProvide access to clinical supervision to all staff as neededAs a member of the Intake Committee, review all applications for admission, participate in home visits to prospective applicants and participate in all admission decisionsProvide clinical support and resources for any psychiatric emergenciesReview and sign support plans, case management summaries, case notes and psychosocial historiesEnsure compliance with contract and regulatory requirements thru regular chart audits to review completeness, timeliness and quality of documentationMeet regularly with Building Manager to review tenant and building issuesCoordinate with Building Manager on staff training and provide resident related counseling to building staff as needed Lead a weekly team meeting with social work staffFacilitate monthly or quarterly meetings with Building Manager and social work staffProvide orientation and training for new social work staffReview incidents and incident reports and submit to contracted agencies as requiredAssume leadership role during site audits by contract agencies; write and submit any required Plans of CorrectionSupervise social work intern/s as neededRequired Skills / Abilities Ability to collaborate effectively with multidisciplinary teams and external partners.Strong organizational, analytical, and problem-solving skills.Commitment to equity, cultural humility, and working with vulnerable and diverse populations.Education & Experience Masters Degree SW required; LCSW preferredMinimum of 2years supervisory experience requiredMinimum of 5 years working with older adults or people with a history of homelessness, chronic mental health issues or substance abuse requiredProven ability to build, foster and lead collaborative groups and workshops for social service staff between various programs Benefit Overview Westside Federation for Senior & Supportive Housing offers full-time employees a comprehensive choice of benefits that include Medical, Dental, Vision, 403(b) retirement savings, Flexible Spending Accounts (FSA), Life & Disability, Employee Assistance Program (EAP), commuter benefits and more.
