What You'll Do

You'll be the face of Preferred Care Home Health in your territory — building and nurturing referral relationships with physicians, hospitals, discharge planners, case managers, social workers, skilled nursing facilities, and assisted living communities.

• Develop and grow referral relationships across your assigned territory
• Build lasting partnerships with physicians, hospitals, and post-acute care partners
• Drive market share growth through strategic territory planning and business development
• Deliver polished presentations that educate referral sources on our services and clinical programs
• Act as the go-to liaison between referral sources, patients, physicians, and our internal team
• Pre-screen referrals for home health eligibility when appropriate
• Manage physician orders through to signed completion
• Stay in consistent contact with referral partners to keep patient care on track
• Represent Preferred Care at networking events, healthcare association meetings, and community outreach
• Keep activity reports and documentation accurate and up to date
• Stay current on Medicare guidelines and home health regulations
• Partner closely with clinical and operations teams for smooth patient transitions

What You Bring

• 2+ years of home health sales experience
• An established book of business and strong local referral relationships
• A track record of meeting or beating sales targets
• Strong communication, presentation, and relationship-building skills
• Self-motivation, solid organization, and time management
• Comfort working independently as part of a high-performing team
• Reliable transportation and a valid driver's license
