Position: Pharmacist – Acute Care
Schedule: Mon–Fri 7:30am–4pm (occasional weekends July+, Sat–Sun 8am–12pm w/ comp day)
Pay: $55-61/hr based on experience
Setting: 35-bed critical illness recovery hospital (post-acute rehab unit within hospital)

This role includes:

• Medication reconciliation (admit & discharge)
• Patient/family discharge counseling
• Pharmacokinetic dosing and monitoring
• Anticoagulation and renal dosing management
• TPN dosing
• Culture review and clinical recommendations (UTI, pneumonia, STI)
• Maintaining pharmacy inventory and emergency procurement
• Paper MARs (no scanning – strong attention to detail required)

Requirements:

• Active PA Pharmacist license
• BLS (AHA preferred or willing to obtain)
• Minimum 1 year recent acute care hospital experience (within last 2 years)
• Experience with TPN, PK dosing, anticoagulation monitoring, renal dosing
• EPIC experience preferred

Optional: On-call available (very light, typically phone-based)
