Pharmacist - Travel Job

Minimum Requirements:

• Active Pharmacist license in the state of Pennsylvania required.
• Experience with hospital pharmacy operations required.
• Experience with sterile product preparation, parenteral labeling, and medication compounding.
• Knowledge of controlled substance handling, record keeping, and regulatory compliance with federal and state pharmacy laws.
• Experience performing medication reconciliation and discharge medication counseling.
• Clinical experience with antimicrobial stewardship, interpretation of culture results, and recommendations for common infections such as UTI, pneumonia, and SSTI.
• Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring.
• EPIC experience required.
• Strong verbal communication skills for gathering prior-to-admission home medication lists and providing discharge counseling.
• Minimum 6 months of pharmacist experience preferred.

Preferred Qualifications

• Previous experience in a hospital or acute care pharmacy preferred.
• Experience participating in Pharmacy & Therapeutics committee activities and medication use evaluations preferred.

Assignment Details

• Shift pattern: 5x8 day schedule.

Impacting Patient Care Nationwide

Provide pharmaceutical care that supports patient safety, medication stewardship, and clinical outcomes. Participate in medication-use evaluations, formulary management, and quality assurance activities. Support safe handling and storage of medications, maintenance of inventory and antidote supplies, and staff competency development. Maintain confidentiality of patient and medical staff information and comply with hospital policies and accreditation standards.
