Discharge Planning Clinician - Care Coordination & Advocacy

SEARHC in Southeast Alaska seeks a Social Services Clinician to lead discharge planning and care coordination for patients from birth to elder. You will join a compassionate, multidisciplinary team to strengthen families and advance tribal health.
