Case Manager Supervisor RFT New Port Richey, FL, US 5 days ago Requisition ID: 1098 Position Summary : This is an entry level management position responsible for the supervision and coordination of quality services and staff of a case management team focused on the provision of case management services in the areas of dependency and adoption. Essential Functions: Responsible for direct supervision of case management team as well as coordination, accuracy, appropriateness, and timeliness of workflow. Must be available as mentor and coach to assist and train team members in the timely preparation and completion of work assignments. May carry out an occasional case troubleshoot and ensure timely monitoring and delivery of services. May require the use of your personally owned vehicle. Ability to document timely in a clear, concise manner, and teach others to do so. Possess skills to assist, interpret, and deliver case and court related information. Possess a positive attitude concerning all aspects of working in a challenging environment including significant patience and respect for peers and team members, as well as the children and families receiving program services. Case Management Certification required. Subject matter expertise in foster care and adoptions. 2+ years of supervisory experience and 2+ years in direct service preferred. Specific job responsibilities and goals include but are not limited to: Supervise day to day operations of one case management team under the direction of Program Director and Assistant Program Director, Case Management Services. Oversee and direct the client data collection and reporting processes, ensuring that services are delivered as designed and outcome measures defined by the program are accomplished. Ensures of cases have a full case review supervision bimonthly and the case manager has individual supervision bimonthly. These will be alternated every other month, ensuring the case manager feels supported and mentored. Participate in projection tracker updates and permanency cohort meetings. Intentionally participates in the retention of staff, treating everyone based on their level of need. Coordinates team on-call cycle ensuring that clients have access to case management and other staff as appropriate. Handles low level crisis calls and situations, informing, and engaging chain-of-command as necessary and appropriate. Provides, as needed, assigns direct services to programs, children, and families. Participates in community groups, meetings, and training related to program issues YFA is involved in at the direction of Program Director and Assistant Program Director. Maintains professional certification by participating in continuing education process. Must understand and demonstrate sensitivity to cultural, community, family, and child needs. Completes other tasks or special projects as assigned. Participate in the on-call rotation. Other duties as assigned by immediate supervisor or leadership. Trained and authorized to access protected health information.
