Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients to identify emotional, social, and environmental needs, considering the unique circumstances of each individual in hospice care.

Patient Population / Client Focus\n

This role specifically supports older adults and terminally ill patients who may be experiencing depression, anxiety, grief, or other mental health challenges related to their impending transition. You will work closely with families, providing not only individual support but also guidance to caregivers navigating the complex emotional landscape associated with hospice care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, equipping you with the foundational knowledge for effective practice.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

Compensation and Benefits\n
