This long-term opportunity as a Home Health Nurse Assessor allows you to conduct in-home assessments that help ensure patients receive the care and services they need. If you are passionate about community-based care, geriatric health, and comprehensive patient assessments , this role offers a rewarding career path with long-term stability. This is a Hybrid set-up, only LOCAL candidates may apply. Rate increase valid for caregivers submitted during May and start in June Position Overview Position: Registered Nurse (RN) – Home Health Nurse Assessor Specialty: UAS Assessment / Home Health Assignment Length: Long-Term Assignment Minimum Requirements Candidates must have at least 2 years of home-based healthcare experience within the last 10 years , including one or more of the following: Field work with a Certified Home Health Agency (CHHA) Work with a Licensed Home Care Services Agency (LHCSA) Experience with Managed Long-Term Care (MLTC) programs Conducting home-based patient assessments (e.g., OASIS) Providing primary care to homebound patients Care management for individuals receiving home and community-based services , hospice, or assisted living Home care experience as an LPN or Home Health Aide caring for at least 2 patients per week Simple, Stress-Free Onboarding Getting started is easy.
