Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children and their families.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP) tailored to the specific goals and priorities of the families served.

• Monitor and evaluate the outcomes of interventions, adapting approaches in response to child development progress and family feedback.

• Facilitate open communication and develop trusting relationships with families, providing essential support and resources to enhance their children’s developmental skills.

• Comply with all health and safety standards to create a safe and nurturing environment for staff and clients.
\n
• Utilize the Natural Learning Environment Practices to ensure learning occurs in a familiar setting.
\n
• Guide families in implementing strategies that promote their child's development, emphasizing coaching and natural environment teaching methodologies.

Patient Population / Client Focus\n

This position focuses on supporting infants and toddlers with disabilities or developmental delays who are at the critical stage of early childhood. Through our services, we aim to equip these children with essential physical, cognitive, social/emotional, and communication skills that are vital for their growth. By working closely with families, you will empower them to advocate for their child's needs and collaborate effectively within the care team.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Must possess a current LCSW license issued by the Department of Public Health.

• A valid driver’s license is required for occasional in-community visits.

Compensation and Benefits\n
