Nurse Practitioner, In-Home Visits

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Responsibilities As a clinician, you will: Conduct in‑home visits lasting about 45 minutes to connect one‑on‑one with patients. Review medication and medical histories, perform physical evaluations and, if ordered by the health plan, conduct additional diagnostic tests such as diabetic eye exams or spirometry. Use an iPad and an intuitive clinical workflow for each evaluation. Not be required to prescribe medicine, order lab tests or alter treatment regimens. Help visit millions of people nationwide and connect them to the right care for their unique needs. Flexibly travel locally and within licensed states as needed. Conduct virtual visits when required. Provide all Signify Health services indicated, including diagnostic and preventive services products (e.g., spirometry, DEE, bone density screening). Qualifications Active, unrestricted license(s) in the coverage area(s) of the role. Board certification (required for NP and PA). Multi‑state licenses are preferred. Eligible to cover primary locations: Hartford, CT. Expected weekly hours: 29.5 (part‑time). Pay Typical pay range: $46.03 – $99.14 per hour. Final offer will depend on experience, education, geography and other factors.
