Nurse Practitioner for In-home Visits

Welcome to Signify Health, part of CVS Health®. We’re hiring board‑certified Nurse Practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focusing on a person’s overall health and well‑being and identify potential barriers to care. Complete a medication and medical history review and a physical evaluation at the health plan member’s home. Use a pre‑populated, patient‑specific form for each evaluation. Do not prescribe medication, order lab tests, or alter current treatment regimens. Compensation and Work Schedule Earn $2,500 to $4,000 per week (average 6‑9 visits daily, $80 per completed assessment). Per‑visit rate increased up to 135% or 160% with Market Multiplier, based on county. Earn additional compensation via pay‑per‑test model when members qualify for diagnostics or preventive tests. Weekly shifts available Monday to Sunday; nights and weekends are not required. Benefits Flat‑rate compensation with potential for higher earnings. Malpractice coverage. Availability incentives based on weekly availability. Optional travel benefits: airfare, hotel, and rental car available depending on eligibility. Incentive rates are subject to change.
