Nurse Practitioner, In-Home Visits

Position Overview Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves visiting patients in the comfort of their own homes, conducting medication and medical history reviews, performing physical assessments, and ordering optional diagnostic tests when directed by the patient’s health plan. Key Responsibilities Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes. Visit millions of people nationwide, providing connections to the right care for each individual’s unique needs. Collaborate with Signify Health as a clinician colleague, fulfilling the responsibilities of the specified role. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits when required. Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, bone density screening, etc. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Eligible to serve as a Nurse Practitioner or Physician Assistant. Compensation and Work Hours Anticipated Weekly Hours: 29.5 Time Type: Part‑time Pay Range: $46.03 – $99.14 per hour (base rate or base annual salary, depending on level). Location Primary coverage area: Hartford, CT. Ability to travel within licensed states as required. Benefits Malpractice insurance coverage during Signify Health‑related activities.
