Nurse Practitioner for In-home Visits

We’re hiring board‑certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on the person’s overall health and well‑being, and identify potential barriers to care. Perform medication and medical history review and a physical evaluation in the member’s home. Use the pre‑populated, patient‑specific form for each evaluation. Do not prescribe medication, order lab tests, or alter the member’s current treatment regimen. Compensation & Benefits Earn up to $2,500–$4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment). Per‑visit rate increases up to 135%–160% with our market multiplier, depending on the county. Additional compensation through a pay‑per‑test model for qualifying diagnostic and preventive tests. Shifts available Monday–Sunday (no nights or weekends required). Short‑term or long‑term on‑call basis. Pre‑scheduled visits in the member’s home. Airfare, hotel, and rental car available when eligible. Malpractice coverage provided. Availability incentives based on weekly availability. Flexible schedule and independent contractor, 1099 status. Qualifications Board‑certified nurse practitioner. State license required. Medicaid license or willingness to obtain one with our support (highly preferred). Independent contractor (1099) status.
