Nurse Practitioner for In-home Visits
$2,500 - $4,000 per weekSignify Health
Job Overview Signify Health, part of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Compensation is based on the number of completed assessments. You can earn up to $2,500 to $4,000 each week (average 6‑9 visits daily, base rate $80 per assessment). Per‑visit rates can increase up to 135% or 160% depending on the work county with a Market Multiplier. Additional pay is available through a pay‑per‑test model for visits where the member qualifies for diagnostic or preventive tests. *Incentive rates are subject to change. Area exclusions apply. Responsibilities Conduct in‑home visits focusing on the patient’s overall health and well‑being. Review medication and medical history and perform a physical evaluation. Identify potential barriers to care. Use pre‑populated, patient‑specific forms; do not prescribe medicine, order lab tests, or alter the patient’s current treatment regimen. Schedule & Availability Shifts available Monday‑Sunday (no nights/weekends required). Short‑term or long‑term basis. Pre‑scheduled visits in the patient’s home. Airfare, hotel, and rental car available depending on eligibility. Requirements Board‑certified Nurse Practitioner. State license required. Independent contractor (1099) status. Flexible schedule; willingness to travel to patients’ homes. Benefits & Incentives Malpractice coverage provided. Availability incentives based on weekly availability. #J-18808-Ljbffr
