The NP provides advanced assessment, diagnosis, and management of patients with serious illness across care settings. This role emphasizes patient- and family-centered care, expert communication, and coordination with the interdisciplinary team to improve quality of life, align treatment with patient goals, and ensure continuity of care.ESSENTIAL JOB FUNCTIONS:Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs.Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.Facilitate advanced care planning and goals-of-care discussions with patients, families, and healthcare providers.Prescribe medications, treatments, and interventions consistent with licensure and scope of practice.Provide clinical leadership and mentoring to the interdisciplinary team.Ensure compliance with Joint Commission, state, and federal regulations.Participate in quality improvement initiatives and program evaluation.Travel required to patient locations throughout the New York metro area.Works within legal, ethical practice standards to his/her position as well as Calvary's Code of Conduct. Comply with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.
