Work Environment

Although the position is primarily remote, our community health clinic operates with a strong collaborative spirit. We encourage flexibility and adaptability in our work environments to ensure we meet the needs of our clients effectively. Regular video conferences and team meetings will help maintain open lines of communication, ensuring you remain integral to our interdisciplinary team composed of healthcare professionals, caregivers, and community resources. This role typically adheres to a Monday through Friday schedule from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed.

Key Responsibilities

• Conduct comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, mental, and social health factors to develop personalized care strategies.
• Collaborate closely with clients and their families to create individualized care plans that reflect their unique goals and preferences.
• Provide ongoing counseling and emotional support to help seniors navigate challenges and transitions in their lives.
• Advocate for clients, ensuring they have access to appropriate community resources and services to meet their specific needs.
• Work in conjunction with nurses and other healthcare professionals to streamline care coordination and ensure a holistic approach to client wellbeing.
• Perform regular home visits to assess clients' living conditions, monitor their emotional states, and respond promptly to any concerns.
• Facilitate communication and problem solving among family members to enhance support systems for clients.
• Educate clients and their families on available community services and programs that can promote social engagement and higher quality of life.
• Maintain thorough documentation of client interactions, assessments, and progress notes, ensuring compliance with ethical standards and organizational policies.

This position primarily focuses on seniors, many of whom may be dealing with mental health challenges, loneliness, and significant life changes. You will be expected to address the unique needs of an aging population, providing a compassionate and informed presence as they navigate their journeys. Qualifications

• A master’s degree in Social Work from an accredited institution.
• Current LCSW licensure in the state of Florida.
• Minimum of two years of experience in social work, preferably in a community health or geriatric setting.
• Strong understanding of aging-related issues and the principles of social work.
• Exceptional communication and interpersonal skills, with an ability to foster working relationships with clients, families, and multidisciplinary teams.
• Proficient in utilizing electronic health record (EHR) systems and other relevant clinical software.

Preferred Experience

• Experience working within a community health clinic or similar environment.
• Demonstrated knowledge of local community resources and support programs for seniors.
• Familiarity with evidence-based social work practices and approaches.

Compensation and Benefits

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience.
