Summary:

\n

The Licensed Practical assists the Registered Nurse in the delivery of patient care. Under the supervision of the Registered Nurse, the LPN is responsible for administering direct patient care, treatments as ordered by the physician and administering medications as outlined by hospital policy and procedure.

Responsibilities:

\n

\n
• Appraises client’s status and situation at hand contributing to comprehensive assessment by the RN.
\n
• Supports ongoing data collection.
\n
• Participates in the planning of nursing care needs and implementation of an individualized plan of care.
\n
• Implements appropriate aspects of care within LPN scope of practice including laws that are applicable to the LPN practice setting.
\n
• Reinforces teaching plan for patients with common health problem and well defined learning needs.
\n
• Completes assignments in an organized, effective, and efficient manner. Provides direct basic care to assigned patients.
\n
• Responds to patient/family needs and question in a timely and appropriate manner.
\n
• Keeps patients/family informed prior to, during, and after treatments/procedures are performed.
\n
• Uses supplies and equipment appropriately; documents use in the medical record and records charges appropriately.
\n
• Complies with time keeping system. Prompt in reporting for duty. Accrues no unexcused absences, and no more than three unscheduled absences.
\n
• Follows nursing policy and procedures in accurate administration of medication, IV therapy, and blood therapy. Recognizes and responds to overt medication reactions. Reports errors to the RN promptly.
\n
• Shares information accurately with supervising RN in timely manner.
\n
• Patient care reflects identification of normal and abnormal signs and symptoms in the patient’s physiological status. Patient care identifies psychosocial needs of the patient and family.
\n
• Normal/abnormal data is reported to the RN in a clear, concise, organized, and in a timely manner.
\n
• Documents observations, treatments, nursing intervention, and patient response in the patient record.
\n
• Follows hospital and/or unit policy for dress and personal hygiene.
\n
• Follows necessary safety precautions, hospital policies, and departmental policies.
\n
• Completes infection control, safety, and other assigned education.
\n
• Follows hospital policies and/or departmental rules.
\n
• Follows the CHRISTUS Health Guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI).
\n
• Associate timely completes all required educational assignments.
\n
• Seeks out new learning experiences.
\n
• Identifies strengths as well as areas where development would be beneficial. Establishes goals with guidance.
\n
• Contributes to an environment that is conducive to the clinical education of nursing students, other health care students and practice setting. Shares information, explains nursing interventions and/or demonstrates hospital procedures.
\n
• Participates in unit quality improvement. Assists with monitoring. Reports problems that interfere with effective functioning of unit to manager/charge nurse.
\n
• Participates in 80% of prescribed in-service programs and departmental meetings. When unable to attend (20% max) follows up to obtain information discussed.
\n
• Participates in hourly rounding and bedside shift reporting
\n
• Demonstrates appropriate care and interventions for the medical surgical patient
\n
• Performs focal assessments in co-ordination with the Registered Nurse
\n
• Collaborates frequently with registered nurse
\n
• Demonstrates the ability to appropriately monitor and provide interventions for actual or potential patient adverse reaction to medications.
\n
• Demonstrates the ability to appropriately perform on-going focal assessment and care for medical surgical patient.
\n
• Demonstrates the proper use of various equipment appropriate to the care/treatment of medical surgical patients.
\n
• Documentation is complete and accurate.
\n
• Identifies and responds appropriately abnormal findings
\n
• Demonstrates appropriate care and interventions for the medical surgical patient
\n

Requirements:

Education/Skills

\n

\n
• Graduate of a State Board accredited Practical School of Nursing or equivalent
  
\n

\n

Experience

\n

\n
• Six (6) months experience preferred.
  
\n

\n

Licenses, Registrations, or Certifications

\n

\n
• Current license or permit to practice professional nursing in the State of Louisiana upon hire
\n
• BLS required
\n

\n
