About the Role:

As a HHA/PCA, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes. Your primary responsibilities will include:

• Assisting clients with daily living activities, such as bathing, dressing, and personal hygiene

• Providing support with mobility, transfers, and ambulation

• Administering medication and treatments as prescribed by a healthcare provider

• Monitoring clients' vital signs and reporting any changes to the supervising nurse

• Maintaining accurate and detailed records of client care and services provided

Responsibilities:

• Provide exceptional care and support to clients in their homes, focusing on their unique needs and preferences

• Collaborate with the interdisciplinary healthcare team to develop and implement personalized care plans

• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

• Adhere to all agency policies, procedures, and regulatory guidelines
