Key Responsibilities\n

• Conduct comprehensive assessments to identify the counseling needs of clients.

• Utilize the DSM (Diagnostic and Statistical Manual) criteria for substance abuse diagnosis and leverage ASAM (American Society of Addiction Medicine) criteria in determining the appropriate level of care for clients.

• Prepare and implement the substance abuse counseling components of individualized treatment plans.
\n
• Develop discharge and continuum of care plans to facilitate seamless transitions to appropriate care levels.
\n
• Maintain meticulous documentation of all counseling sessions, assessments, and follow-up actions within each client's clinical record.
\n
• Manage an active caseload while ensuring compliance with treatment planning and clinical charting requirements.
\n
• Coordinate support services for clients and their families, ensuring that these services are accurately recorded.
\n
• Engage with referral sources such as legal entities, educational institutions, and employers, maintaining open lines of communication for client care.

• Facilitate interventions for clients experiencing crisis situations, employing a collaborative multidisciplinary team approach.
