Key Responsibilities\n

• Conduct comprehensive assessments to identify the mental health needs of patients and develop tailored treatment plans.

• Effectively communicate with supervisors, family members, and other caregivers to provide updates on clients' progress and any necessary adjustments to their treatment plans.

• Maintain an organized schedule of appointments, home visits, and observations for client monitoring and support.

• Coordinate necessary home visits to facilitate in-home observations, chart reviews, and staff interviews as part of the ongoing assessment process.

• Oversee and ensure the effective implementation of treatment plans to meet the individual needs of each patient.

• Participate actively in Interdisciplinary Team Meetings, contributing insights and collaborative strategies for patient care.

• Document monthly and quarterly progress notes to track patient progress and treatment outcomes appropriately.

• Facilitate training sessions for staff and team members to ensure adherence to treatment protocols and documentation standards.

• Provide follow-up services through interviews and home visits, ensuring clients receive continued support in their mental health journey.

• Keep meticulous records, making information accessible to relevant parties to foster continuity of care.

Qualifications\n
