Position Summary Physical Therapist – Outpatient Rehabilitation – Miller, Wake Forest Baptist Medical Center, Winston Salem, North Carolina. Qualifications Licensed to practice Physical Therapy in the State of North Carolina; current licensure required. Bachelor of Science degree from an accredited physical therapy school required. CPR certification required every two years. Active member of APTA encouraged, but not required. Responsibilities Provides patient care assessment and treatment, ensuring evaluations and treatments are comprehensive, functional and aligned with the Standard of Practice. Maintains accurate patient documentation, completing written evaluations, progress notes, and discharge notes per policy, including charges, departmental statistics, and ICD-9 codes as appropriate. Meets department financial and productivity standards, adjusting schedules as needed to meet patient and department needs. Participates in process management to optimize quality and efficiency of services and identifies quality and operational opportunities. Promotes professional development and education of other health care workers and provides representation on committees; conducts in‑service training programs to share state‑of‑the‑art information and enhance safety. Supervises physical therapy and physical therapy assistant students to develop clinical skills and ensure quality patient care. Communicates professionally and timely with patients, physicians, staff, and external customers. Provides appropriate patient care in accordance with age/developmental guidelines, demonstrating age‑specific knowledge, skills, and abilities. Adheres to general hospital standards, fostering a cooperative work environment through communication skills, interpersonal relationships and team building.
