Case Manager Division : HOPICS Full-time The Case Manager will perform a full range of targeted and/or general case management that is strength-based, needs-based, community-based, and family-driven. The Case Manager is responsible for development and implementation of client action plans, monitoring progress, and ensuring services are delivered in line with client goals.
