Housing Case Manager - Families & Rehousing
Upward Bound House
Upward Bound House seeks a BFH Housing Case Manager to work with families in the child welfare system, identifying housing opportunities and guiding rental assistance programs. You will educate landlords on UBH subsidies and help tenants navigate rights and responsibilities. You will conduct comprehensive intake, develop Housing Case Plans, coordinate services, recertify every 3 months, document progress in HMIS, and support participants and landlords through housing challenges. #J-18808-Ljbffr Upward Bound House
