Case Management Registered Nurse

HCA Healthcare is seeking a Case Management Registered Nurse to coordinate safe, efficient patient care transitions. In this role, you will assess patient needs, develop discharge plans, and collaborate with physicians, interdisciplinary teams, patients, and families to optimize clinical and financial outcomes. The RN will manage utilization review, ensure appropriate level of care, and align care plans with payer requirements. You will advocate for patients, arrange post-acute services, maintain accurate documentation, and support quality and readmission reduction initiatives while promoting HCA’s culture of compassion, teamwork, and continuous learning.
