Key Responsibilities\n

• Conduct comprehensive assessments of each client's counseling needs, identifying specific areas of concern such as substance use and mental health issues.

• Evaluate clients for substance use disorders using DSM criteria, determining the level of care needed based on ASAM guidelines to create individualized treatment plans.
\n
• Design the substance abuse counseling segment of client treatment plans, ensuring that approaches are tailored to meet each patient’s unique needs.
\n
• Develop detailed discharge and continuum of care plans to facilitate a smooth transition to subsequent levels of care, supporting long-term recovery outcomes.
\n
• Maintain thorough documentation of all counseling sessions, assessments, and follow-up actions in the client’s clinical record, ensuring compliance with regulatory standards.
\n
• Manage a caseload of clients, including regular case management and utilization review updates, to track progress and adapt treatment plans as necessary.
\n
• Coordinate support services for clients and their families, documenting these services within their clinical records to provide a holistic support network.
\n
• Engage with referral sources such as legal, educational, and employment representatives to promote client engagement and support their recovery journey.

• Utilize a multidisciplinary team approach to facilitate interventions for crisis situations, ensuring that clients receive timely and effective care.

This position involves working primarily with individuals who are experiencing substance use disorders and may have co-occurring mental health issues. Qualifications\n

Compensation and Benefits\n
