Desired Qualities of a Case Manager Positive communication Excellent supportive counseling skills Compassion Reliability & dedication Desire to make a difference Sound ethical and moral values Professionalism Conflict resolution Essential Duties and Responsibilities of a Case Manager Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc. Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing Provide transportation as needed to assist with child care, grocery shopping and other needs in the community. Support families personal outcomes to enhance quality of life Utilize an electronic health record (EHR) system to write clear and concise progress notes Ability to pass all Agency and program specific trainings Training All employees are required to complete 10-15 trainings upon hire and on an ongoing basis. Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting. All trainings are paid.
