- Board Eligible (within 5 years of residency) or Board Certified in OB/GYN - REQUIRED
- CA license (or willing to obtain) - REQUIRED
- OB Hospitalist work experience - PREFERRED
- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
- Is provider interested in full-time or part-time employment? (note in "Additional Submission Details" field) - REQUIRED
- Does provider already live in the area or they willing to relocate? (note in "Additional Submission Details" field) - REQUIRED
