Case Management Coordinator

Randstad Healthcare is hiring for a Medical Case Manager in Miami Beach, FL (zip codes: 33139, 33140, 33141). Job Summary We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our team. The role promotes autonomy through a Monday‑Friday (8:00 AM – 5:00 PM) schedule and flexibility as the Coordinator coordinates the care of our members. Responsibilities Telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case management activities with members to evaluate medical needs and facilitate overall wellness. Effectively manage a caseload that includes supportive and medically complex members. Develop a proactive course of action to address issues presented and enhance short‑ and long‑term outcomes. Determine appropriate services and supports for members’ health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources and supports. Travel Requirements 50‑75% travel is required for face‑to‑face visits with members in Miami Beach, FL (zip codes: 33139, 33140, 33141). The candidate will travel approximately 75% of the time within Miami‑Dade County, seeing members at their homes, in Assisted Living Facilities, and Skilled Nursing Facilities. Qualifications Bilingual in Spanish and English, fluent speaking and writing. Ability to multitask, prioritize, and effectively adapt to a fast‑paced changing environment.
