About the Position

Job Title: RN Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Part-time (approx. 2 face-to-face visits per week) Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years. Recent experience conducting comprehensive assessments of patients is required. License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Candidates must be capable of conducting hybrid Tele and in-field assessments. - Candidates cannot perform UAS assessment assignments elsewhere. - Candidates must sign the COI form before starting. Description: Urgent need for part-time home health RN. Candidates will perform initial in-home pre-evaluations of patients. Evaluations will include assessing health status, strengths, care needs, and preferences. These assessments will guide the development of individualized long-term care service plans. Candidates will review consumer medical documentation and health referral forms relevant to each case. Evaluation data must be entered into an electronic evaluation form and transmitted as required. Any concerns or conflicting information that arise during assessments must be documented. Emphasis on continuity of care is essential to reduce fragmentation and gaps in treatment plans. Candidates will discuss health care options, supports needed, service vendor options, and waiver options with consumers. Communication between the Call Center and eligible consumers for enrollment into managed care health plans must be initiated. Candidates will enhance communication and collaborative relationships with interdisciplinary care team members. Improved care coordination and service delivery are key responsibilities. Quality review data and any required documentation to support outcome measurements must be collected. Case notes should be meticulously recorded in the CRM. Candidates will identify opportunities for health promotion and illness prevention. An understanding of community resources, payor requirements, and network services for the target population is essential. All standards for this position must be consistently met, as outlined in performance criteria and bonus templates. Additional duties may be assigned by the Regional Nurse Manager or other project management.
