Remote Care Manager — Chronic & Behavioral Health
Vitability Health
Vitability Health is a leader in remote care delivery, seeking a Care Manager to provide Chronic Care Management and Behavioral Health Integration services remotely to patients in their homes and assisted living facilities.
In this role you will coordinate care, monitor patient health, and guide self-management with SMART goals, while collaborating with physicians and care teams to ensure timely follow-ups and documentation.
