Nurse Practitioner - Post Acute Care - North Region

Position Highlights Position: Nurse Practitioner (NP) – Post Acute Care Location: Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital) Full Time/Part Time: Full‑Time, 40 hours per week Hours: Monday – Friday between 8:00 a.m. and 5:00 p.m. Potential for weekend hours in the future No evenings or holidays What you will need License: Nurse Practitioner (APN) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required Education: Completion of Master’s level program in specialty area Certification: BLS/CPR certification required Experience: Minimum of three years Nurse Practitioner experience. Experience with advanced directive and goals of care.
