Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population, either in private residence or in a patient's room in an independent living facility. Develop and deliver physical therapy programs to facilitate rehabilitation. Make referrals to other disciplines to address comprehensive patient needs. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Instruct, educate and train patients and caregivers in essential skills and functions, promoting independence and productivity. Supervise Physical Therapy Assistants, temporary licensees, students and support personnel per state licensure requirements and professional standards. Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation, and work‑related meetings. Attend and contribute to patient care, staffing conferences and related meetings. Consult with rehabilitation team members to select appropriate therapeutic regimens. Adapt program and/or plan of care according to patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Cleans, maintains, and reports repair needs for equipment. Put patient service first: ensure patients and families receive highest quality service in a caring atmosphere. Screen, examine, and evaluate patients, including history, systems review, and appropriate tests, to establish diagnosis, identify impairments, predict improvement, determine precautions, and design care plan. Develop treatment goals and methods, collaborate with patient and caregivers, implement the plan, and complete all related documentation. Provide inclusive services such as therapeutic exercise (aerobic conditioning, balance coordination, agility, body mechanics, postural stabilization, flexibility, gait, neuromotor development, relaxation, strength, power, endurance), functional training, manual therapy, airway clearance, integumentary repair, electrotherapy, physical agents, mechanical modalities, health and wellness, chronic disease management education, fall assessment and interventions, case management. Design, fabricate, fit, or apply adaptive devices, and train patients in their use including orthotics and prosthetics. Select appropriate activities based on patient capacity, mental capacity, intelligence level, and interests. Instruct patients in mobility skills, evaluate progress, attitudes, and behavior. Perform routine re‑examination as needed to modify or progress treatment plan. Incorporate health and wellness programming into treatment plans. Conduct home and community mobility assessments and recommend safe mobility. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or a Master’s degree in a related field with a Bachelor’s degree in Physical Therapy, or a Bachelor’s degree in Physical Therapy with five years of progressively responsible PT experience. Foreign‑earned Physical Therapy degree evaluated as U.S. equivalent. Valid driver’s license or reliable independent transportation to patient residences.
