The Health Screener conducts dental, hearing, vision, and wellness screenings throughout Santa Clara County. This role also processes screening results and provides administrative support to the Screening Department. This is an entry-level position, ideal for individuals with a growth mindset and a desire to learn. This role involves working with children and youth from ages 0-18 and requires the ability to build rapport, maintain a positive attitude, and engage with young people effectively.

Essential Functions & Position Responsibilities:

• Conduct physical and wellness screenings at various locations throughout Santa Clara County.
• Provide administrative support for processing dental, hearing, vision, and wellness screening results.
• Provide support to contractors and volunteers in conducting screenings.
• Process screening results in our database.
• Conduct community outreach and provide program presentations.
• Primary liaison for onsite screenings.
• Gather feedback from stakeholders.
• Perform other duties as required by the needs of the organization.

Additional Functions & Position Responsibilities with CHW Certification:

• Act as liaison for individuals to access health care providers and community resources.
• Schedule health and wellness appointments, preventative care, promoting the use of Primary Care Provider (PCP) and the patient’s medical home.
• Establish rapport with individual seeking services, practicing consistent follow-up, trust-building, and empathy skills by sharing responsive and accurate community resources.
• Assist health plan members in accessing preventative health services and provide intensive coordination of health and health-related services for highest-need members. This includes working closely with healthcare staff and managing client cases.
• Conduct outreach activities to inform community members about available health services and preventative health services

Minimum Requirements:

• High school diploma or equivalent.
• Community Health Worker (CHW) Certification, obtained within 6 months of start date.
• Fluent in Spanish
• Excellent Computer skills (Microsoft Office Software including Word, PowerPoint and Excel).
• Maintain confidentiality at all times.
• Participation in all agency events is required.
• May be required to work occasional evenings and weekends.
• Must have valid driver’s license and reliable transportation.

Shift & Schedule:

• Expected hours between 8 – 30 hours per week
• 4 – 6 hour shift
• Day Shift
• Monday - Friday
• Weekends as needed
• Year-round work
• The hours for this role may vary depending on business needs and may not guarantee a consistent 8-hour workday.

Benefits:

Benefits begin first of the month following date of hire. To qualify for benefits, you must work a minimum of 20 hours per week on average.

Employees who work between 20 and 31 hours per week on average are eligible for benefits at a prorated rate (50%), including medical, dental, and vision coverage, 403b retirement plan, PTO, and partial coverage of phone expenses.
