Job Highlights Worker Type: Regular Department: Specialty Transplant Clinic Patient Population: Transplant – liver, kidney and GI Schedule: Monday – Friday 8:00 am – 4:30 pm Location: SSM Health Saint Louis University Hospital Job Summary The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates planning with other disciplines using a patient–customer driven approach. Responsibilities Comprehensive Nursing Assessment and Reassessment Perform age‑appropriate admission or transfer assessment and obtain family input when appropriate. Accurately and completely document findings. Assess post‑operative/post‑invasive procedure patients. Asses and document education and discharge needs of patient and family on admission and throughout hospitalization. Provide patient reassessment documenting pertinent observations according to the patient plan of care and changes in condition. Plan of Care Development and Implementation Establish, coordinate, and evaluate a plan of care based on assessment data, patient diagnosis, lab data, procedures, physician orders, protocols and standards of care. Identify short‑ and long‑term goals based on patient care needs. Formulate nursing interventions to achieve desired outcomes. Incorporate disease‑specific evidence‑based practice into the care plan. Document and communicate nursing interventions and patient status changes. Care Coordination and Patient Communication Collaborate with health‑team members for daily care plan coordination. Provide, coordinate, and communicate patient care, including accurate hand‑off communication reports. Administer and document medications accurately according to policies, and document IV fluids and parenteral nutrition. Complete referrals as indicated by assessment data and request consults for special needs. Provide patient/family education and discharge planning per protocol. Clarify and immediately implement physician orders. Assist physicians with procedures or delegate to Care Partner when appropriate. Document telephone/verbal orders accurately; limit such orders to emergencies. Respond to patient condition changes and involve family/guardians in decision‑making. Apply infection‑control practices daily. Evaluation and Documentation of Care Effectiveness Ongoing evaluation of care effectiveness based on assessment, interventions, medications and patient response. Evaluate effectiveness of patient/family education. Modify the plan of care as indicated. Report significant changes in patient clinical parameters immediately to physician and team. Identify problems, gather data, suggest solutions, and pursue resolution. Report variations from care/treatment per policy. Specialized Care for High‑Risk Patients Restraint Care Initiate or evaluate alternatives to restraints before application. Apply restraints according to approved procedure. Monitor and assess patient response at intervals. Provide specified patient care (toileting, skin care, hydration, feeding, etc.) timely. Consult with peers for restraint alternatives and 1:1 observation. Document restraint use and associated care. Pain Management Assess pain presence on admission and during assessments. Incorporate cultural/spiritual beliefs into pain plan. Implement preventive pain‑management techniques. Include patient/family in pain plan development. Consider massage, repositioning, immobilization, and music therapy. Abuse Assessment Recognize abuse criteria and incorporate into assessment. Report signs of abuse/neglect to appropriate authorities. Take actions to support patient safety. Professional Accountability and Development Adhere to quality and performance standards and policies. Participate in learning experiences that increase competence. Maintain currency in unit policies, procedures, and communications. Attend staff meetings, participate in committees, shared governance, and unit‑based practice councils. Adapt positively to changing demands and workload; provide support to peers. Orient new employees and maintain a collaborative team approach. Supervise and teach students/new staff nurses. Perform duties safely, maintain infection control, and keep environment safe and clean. Maintain mandatory certifications and trainings. Patient-Centered Service Excellence Provide prompt, respectful, courteous service. Meet physical, spiritual, psychological, and emotional needs of patients and families. Answer call lights promptly; apologize for delays; communicate effectively while maintaining confidentiality. Provide accurate hand‑off reports and quality care per standards. Use resources wisely; recommend practice changes to increase efficiency and reduce waste. Apply the hospital’s AIDET principles and One Mission, One Spirit commitments. Performance Improvement and Evidence‑Based Practice Assist in developing and implementing unit and hospital performance improvement plans. Participate in data collection, propose solutions, and implement changes. Lead performance improvement teams when requested. Remain current with evidence‑based practice initiatives. Participate in nursing research and quality indicator improvement. Qualifications Licensed Registered Nurse (RN) in the state of Missouri (or other applicable state as per license table). Basic Life Support (BLS) or equivalent certification. Proficient in use of Electronic Health Records (EHR) and clinical documentation. Experience in transplant or critical care nursing preferred (liver, kidney, GI). Strong communication, critical thinking, and teamwork skills. Ability to work shift hours Monday‑Friday, 8:00 am‑4:30 pm, with willingness to support urgent care needs. Job Specifications Minimum Education: Registered Nurse (RN) license in Missouri. Working Conditions: Exposure to blood‑borne diseases, accidents, injuries, and illness; irregular hours; emergency and crisis situations. Physical Requirements: Standing, walking, lifting, pushing, patient transfers, reaching, stooping, bending, kneeling, crouching.
