The Transitional Care Coordinator manages cross-continuum care episodes and provides ongoing assessments of patient and family needs via telephonic and in-person interactions.

Responsibilities

•Identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use. Encourage patient and family engagement with disease and self-management through identification of new and existing barriers.

•Assist with process improvement, care integration, and a primary focus on reducing hospital readmissions.

•Consult and educates the patient/family and other caregivers regarding the disease process, pain and symptom management, end of life care and processes when appropriate.

Qualifications

•A graduate of an accredited school of nursing.

•Prior to date of hire, must hold a current, valid RN license or temporary RN license, or hold a compact RN license. Must maintain valid licensure to practice as a RN.

•CPR required

•Minimum of three (3) years’ experience in a healthcare setting required.

•Must have experience with medical case management.

•Valid driver's license with maintenance of safe driving record and an automobile that is insured in accordance with state requirements required.
