Position Summary

The Case Manager provides care coordination and support services to clients receiving behavioral health and substance use disorder treatment services within a trauma-informed treatment environment. This position assists clients with accessing community resources, coordinating referrals, supporting discharge planning, and promoting continuity of care throughout treatment.

The Case Manager works collaboratively with the multidisciplinary treatment team to support client stability, treatment engagement, and successful transition planning. This position maintains compliance with applicable regulations, organizational policies, and professional ethical standards while promoting a safe, supportive, and client-centered environment.

Duties and Responsibilities

· Assist clients with accessing community resources including housing, transportation, medical care, benefits, behavioral health services, and recovery support systems.

· Coordinate referrals, appointments, discharge planning, and continuity of care services.

· Maintain communication with referral sources, community providers, outside agencies, and treatment team members as appropriate.

· Assist clients with identifying goals, barriers, and support needs related to treatment and recovery.

· Participate in multidisciplinary treatment team meetings, staffing discussions, and discharge planning processes.

· Support clients with problem-solving, resource coordination, and community integration.

· Maintain timely, accurate, and complete documentation in accordance with Medicaid standards and organizational policies.

· Monitor client progress and communicate updates to the treatment team as appropriate.

· Support crisis intervention efforts and assist clients with stabilization and resource coordination when needed.

· Advocate for client needs and assist with identifying barriers to treatment, recovery, and community integration.

· Perform related duties as assigned.

Position Requirements and Expectations

· Experience working in behavioral health, substance use disorder treatment, healthcare, or human services preferred.

· Knowledge of community resources, discharge planning, and care coordination practices preferred.

· Proficiency in maintaining accurate and timely documentation within the Electronic Health Record (EHR) system.

· Ability to maintain professional boundaries and adhere to ethical standards of practice.

· Ability to work collaboratively within a multidisciplinary treatment environment.

· Valid Alaska driver's license.

· Current CPR and First Aid certification.

· Ability to successfully pass all required state and federal background screenings.

Preferred Experience

· Experience working with co-occurring mental health and substance use disorders.

· Experience with discharge planning, referral coordination, and community-based services.

· Knowledge of Alaska Medicaid systems and behavioral health resources.

· Experience working within trauma-informed and recovery-oriented treatment environments.

Education Requirements

· Bachelor’s degree in Behavioral Health, Social Work, Human Services, Psychology, or related field preferred.

· Relevant experience in lieu of education can be considered.
