Responsibilities

• Provide direct, patient-centered nursing care to home health patients within the assigned area, including planning and delivering evidence-based interventions.
• Assess patient conditions, monitor vital signs, administer prescribed medications, manage wound care, and implement care plans in collaboration with the interprofessional team.
• Document assessments, interventions, and outcomes accurately in the medical record; ensure compliance with regulatory and payer requirements.
• Educate patients and families on disease management, medications, equipment use, and safety to promote optimal functional outcomes and self-care.
• Coordinate with supervisors, physicians, and case managers to adjust plans of care as patient needs evolve; facilitate transitions across care settings when appropriate.
• Maintain a safe and therapeutic environment, adhere to professional standards and agency policies, and participate in performance improvement activities.

Requirements

• Active Registered Nurse (RN) license in the state of practice (California required)
• CPR/BLS certification required (BLS, HS-BLS, or RQI-BLS accepted)
• Valid Driver’s License and must be at least 21 years old
• Ability to work in a home health/hospital setting as a travel RN
• Home health or acute care experience preferred
