Work Environment

This position is predominantly onsite, working within the homes of our patients throughout Daytona Beach and surrounding areas. By delivering therapy in a home care setting, you will directly assess and address the unique challenges faced by each patient in their own environment. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, assessing their strengths, needs, and rehabilitation goals.
• Design and implement individualized treatment programs that reflect best practices and evidence-based methodologies across various physical therapy disciplines.
• Deliver high-quality, purposeful therapeutic care directly in patients' homes, ensuring that each session aligns with the physician's plan and enhances the patients' functional abilities.
• Educate and empower patients and their families, engaging in discussions that help them comprehend treatment goals, procedures, and expected outcomes.
• Maintain accurate and timely documentation of patient care, progress notes, and treatment outcomes to comply with clinical requirements and ensure continuity of care.
• Foster an environment of outstanding customer service, working collaboratively with a multidisciplinary team to enhance patient satisfaction and achieve optimal recovery outcomes.
• Meet established utilization metrics and contribute to the overall operational efficiency of the practice.

Patient Population / Client Focus

You will have the opportunity to work with a diverse array of patients, including those recovering from orthopedic surgeries, pediatric patients, individuals with neurological conditions, and athletes requiring sports medicine interventions. Understanding the specific needs and rehabilitation goals of each patient will be essential in providing tailored therapy that promotes wellness and functional independence.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program (APTA or CAPTE-approved).
• A valid state license to practice physical therapy in Florida.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong written and verbal communication skills, complemented by computer proficiency, including electronic health record (EHR) systems.
• A proven ability to build rapport and establish meaningful connections with patients, enhancing dynamics that lead to successful rehabilitation.
• A demonstrated passion for physical therapy and a desire to contribute positively to the health of the community.

Preferred Experience

• Previous experience in in-home care settings or outpatient physical therapy environments.

Compensation and Benefits
